Levels of Care
Residential Care

For those residents that are living independently but would also like
access to medical care if or when necessary. Residential care provides supportive services like meals, housekeeping, and transportation as well as fitness programs and social engagements. Residents at this level should be able to attend to all personal needs without assistance or oversight from the staff. Nursing services are available when necessary.

Level I

This level of care is intended for those who require only minimal assistance from
staff. These residents are able to attend to most personal needs without assistance, including bathing and dressing. Typical services at this level include medication administration, bed making, biweekly housekeeping, encouragement to attend planned events and general oversight of medical care.

Level II

Residents at this level require moderate support from Warwick Forest’s caring and committed staff. This includes some assistance with dressing and mobility and occasional cueing or redirection for those with cognitive disorders. Residents at this level might also have conditions that require monitoring by licensed professional nurses. Occasional medical treatments and dressing changes, for example, are included at this level.

Level III

Services for residents at this level include all Level I and Level II care, as well as more extensive assistance from our staff. Increased frequency of monitoring and multiple nursing services are included, and Level III might also include managing residents with incontinence as well as those with earlier-stage memory impairments.

Because no two residents are alike, neither are our plans of care. Within each level, residents are treated with dignity, respect and compassion. Our professional staff will conduct regular assessments and stay in close communication with loved ones to ensure each resident receives the services they need to stay healthy and independent.

At Warwick Forest Assisted Living, monthly rates are based on your loved one’s selected floor plan and individualized level of care. The ranges below are provided to help you begin planning with clarity and confidence.

Residential Care Plans: $4,985 - $7,218*
Levels of Care: $147 - $2,237
