Living With Dementia Is a Journey

Nearly 6.7 million Americans live with dementia or some form of cognitive decline. The GUIDE Model was designed by The Centers for Medicare & Medicaid Services (CMS) to connect caregivers and those living with dementia with essential resources, support and education within their community and partner with them throughout their journey.

Personalize Care Plans
You will have a care plan tailored to meet your specific needs and goals.

Regular Check-Ins
Your care navigator will regularly check in with you by phone, virtual visits or in-person appointments.

Medication Management
You will have support to ensure that you are able to maintain the goals of your medication plan.

24/7 Support
Urgent concerns can be addressed 24/7 through access to a dedicated support line.

Identification of community-based resources and social engagement opportunities.

GUIDE Model Program Eligibility

GUIDE Model Program Eligibility

To be eligible for the GUIDE Model, participants must be community-dwelling* and meet the following criteria:

  • Have a confirmed diagnosis of dementia
  • Be enrolled in Medicare Parts A and B and have Medicare as their primary payor
  • Not enrolled in Medicare Advantage
  • Not enrolled in the Program of All-Inclusive Care for the Elderly (PACE)
  • Not enrolled in Medicare hospice

* “Community-dwelling” is defined as living in a personal home, assisted living facility, group home or other community setting and excludes long-term nursing home residents.
