Phase III Cardiac Rehabilitation

Phase III is a part of Riverside's continuum of care after the graduation of outpatient Phase II Cardiac Rehabilitation. It is designed to provide education on reducing the cardiovascular disease risk factors, creating a realistic exercise regimen, and improving their overall quality of life.
