In response to growing community need for specialized care for older adults, Riverside Health is bringing its Geriatric Assessment Clinic to Riverside Eastern Shore Family Medicine on August 28 and 29, 2025.
The clinical team is based out of Martha W. Goodson Center in Williamsburg, the Geriatric Assessment Clinic is expanding its reach for two days to serve residents on the Eastern Shore where access to geriatric and Memory Care specialty services can be limited by geography and travel.
“Bringing this clinic to the Eastern Shore reflects Riverside’s commitment to delivering expert, compassionate care—where and when it’s needed most,” said Tina Thomas, Executive Director of the Martha W. Goodson Center. “Our goal is to meet older adults and their caregivers where they are, providing tools to support independence, well-being, and peace of mind.”
Led by a board-certified geriatrician and supported by an interdisciplinary consultative team, the clinic provides comprehensive outpatient assessments tailored to the unique health concerns of older adults. Services include:
- Memory screenings: looking for signs of memory loss, brain changes, or depression
- Health issues: reviewing things like falls, nutrition, incontinence, or trouble sleeping
- Medication reconciliation: making sure medications are safe, accurate, and not causing side effects
- Mental health, mood, behavior, safety concern: watching for changes and offering care suggestions
Patients may also be referred for lab testing, CT head scan, or neuropsychological evaluation, based on the outcome of the initial visit. When appropriate, referrals to Riverside’s Memory Care Navigation program are made to offer continued guidance for patients and caregivers managing memory loss or dementia-related concerns.
Following the assessment, the team delivers clear, individualized recommendations for managing current concerns, along with resources and strategies to support long-term health planning for patients and their families.
