Welcome to the Riverside Eastern Shore Family Medicine. We provide comprehensive primary care services for adults and older children.
Appointments
To make an appointment, please call our office. Please note that we do not accept walk-in patients. If you need to change your appointment time, we appreciate notification at least 24 hours in advance of your scheduled visit.
Please arrive thirty minutes early for your first appointment so that we may get to know you and obtain important health information.
What to bring to your first appointment:
- Insurance card
- Photo I.D.
- Co-payment, if applicable
- List of medications and dosages or the original prescription bottles
If you have been or are currently being treated by a physician of Riverside Medical Group, we can access your medical records electronically. If not, and if at all possible, please bring copies of relevant medical records.
Insurance
We accept most major insurance including Anthem, Cigna, Choice, Optima, United Healthcare and Virginia Health Network among others. We are also accepting new Medicare patients. When you call to make your first appointment, please verify that we accept your insurance.
Medical records
As a means of expediting care and increasing patient safety, we store medical records electronically. This highly-secure system facilitates fast, clear communication among your physicians. In the event of an emergency, our emergency physicians have immediate access to your medical history. This information can be critical and sometimes lifesaving in helping to properly diagnose and treat your condition. Please be assured your personal medical records are confidential, and we will not release information without your permission. Review Riverside Health's Privacy Statement for more information.
Manage your health online
We've introduced a new electronic health record, which you can access via Riverside MyChart. You will have 24/7 access to parts of your health record by enrolling in MyChart .
Riverside MyChart has many new features and enhanced functionality that will allow you to:
- Communicate with your doctor
- Read open notes about your visits
- Access your test results
- Request prescription refills
- Manage your appointments
- View bills and pay online
- Manage your loved one's health through proxy access

This practice serves all patients regardless of ability to pay. Discounts for essential services are offered based on family size and income.

Insurance
We accept most major insurance. If you change insurance and need assistance, please call our office at 757-442-6600.
Medicare
Becoming insured through Medicare can be confusing. We recommend reading about the program at the government's official site www.medicare.gov. Feel free to call our staff with any questions.
Check-up guidelines
One of the keys to lifelong good health is to have regular health screenings. The National Institutes of Health recommends these medical screenings based on your age and gender.
Riverside Nurse telephone service
What can you do if you develop a cough at midnight? Call Riverside Nurse, a free service that's available 24 hours a day, 7 days a week. A registered nurse trained in telephone triage will answer your questions and, if needed, refer you to a physician for treatment. Call 1-800-675-6368. Of course, if you have a life-threatening situation, call 911.
At Riverside Medical Group, we know managing all your care needs, from routine health maintenance to advice from specialists and changes in medications can be complicated, but it doesn’t have to be.
Chronic Care Management (CCM) is a new program available to Medicare patients living with two or more chronic conditions, such as arthritis, diabetes, depression or high blood pressure.
Care Coordination
Riverside Medical Group offers a service that can help you navigate these challenges. Our CareHarmony care coordination service is designed to help you take care of you! You will benefit by receiving monthly telephone assistance in between your regular appointments. This includes a dedicated Care Coordinator to guide you toward better health.
Chronic Care Management extends your care outside of the four walls of your physician’s practice to ensure that you maintain the best possible health. Each month your Care Coordinator will call you to:
- Assist with scheduling appointments, lab tests or other tests
- Explain how and when to take your medications
- Coordinate any home health or medical equipment needs
- Connect you with health education resources, services and programs
- Identify available community resources
- Coordinate follow-up care after leaving the hospital or the emergency room
- Access other support services you need to stay well
There is also a 24/7 Care Coordinator Phone Line to help answer any questions you may have day or night outside of your calls with your Care Coordinator.
Cost and Coverage
This service is covered by Medicare and is subject to your annual deductible and coinsurance (usually 20%). If you have a secondary or supplemental insurance plan, your coinsurance may be covered. It is best to review exactly what your insurance plan covers.
While you can only sign up for Chronic Care Management services with one of your CareHarmony care coordinators, you are able to opt-out any time—there are no strings attached.
