New Hire Assessment Appointment

Your next step towards becoming a member of our team is the completion of your New Hire Assessment Appointment that will include a pre-employment physical and human resources processing. If you will be late or need to cancel, please contact Talent Acquisition ASAP at 757-537-5627 option 3. Please make sure to arrange child care arrangements if needed. This appointment should take less than an hour. If you are younger than 18 years of age, ask your parent or guardian to sign the consent for post offer, pre-placement test. 

Step 1 – Employee Health and What you are REQUIRED to bring:

  • Current Photo Identification, such as driver’s license, Real ID, or Military ID
  • A copy of your immunization record- You can obtain from your Primary Care Doctor, local Health Department, or if resident of Virginia you can obtain your record from the Virginia Immunization Information System. Contact them at:
  • A full bladder for your urine analysis drug screen (recommend drinking at least one hour before your appointment of 16 ounces of water)
  • If your position requires a N95 mask fit test, You must be clean shaven with no more than 24 hours facial hair growth. (If not clean shaven, you will have to return to complete and likely delay your start date.)

Step 2 – What to expect at your physical exam - Estimated time is 1 hour
  • Check in at front desk - Provide completed paperwork
    • Print out completed paperwork and copies of Immunization record
    • You can email all forms prior to appt to [email protected]
  • Covid screening verbal questions will be asked, if you feel ill, please call your recruiter to reschedule your appointment.
  • RN will perform the following
    • Urine Drug Screening
    • Color Blind testing
    • Vision testing ( if you wear glasses for distance, please bring with you)
    • Blood draw for Tuberculosis screening and Titers for vaccinations
    • Vaccinations as needed for job
    • Review of job description physical requirements
    • Fit testing for N95 mask if needed for your job
    • Vital signs check to include blood pressure, pulse and oxygen saturation levels
