Memory Care Navigation

Memory care navigation supports people living with dementia and their caregivers. Living with dementia is a journey. Riverside's free Memory Care Navigation program is a specialized service that provides each individual and their caregivers a personalized approach to care. Call 757-220-4751 to learn more.

Talk to a Riverside senior care navigator

Our senior care navigators can help connect you with resources. This is a free Riverside service. Senior care navigators are also available by phone at 757-856-7030, Monday to Friday, from 8:30 a.m. to 5 p.m.

Get help anytime through Riverside Nurse

Riverside Nurse is available 24/7 to answer your symptom-based question and, if needed, refer you to a physician for treatment. Riverside Nurse also provides general health information and Riverside physician and service referrals. Call 800-675-6368.

Riverside's Center for Excellence in Aging and Lifelong Health taps into the latest research to develop innovative programming. One of the resources CEALH provides is the Geriatric Assessment Clinic.

VirginiaNavigator provides aging and disability resources

VirginiaNavigator is a nonprofit organization that provides free information about health, aging, disability and post-military resources available to Virginians. The information focuses on issues such as health, financial concerns, legal questions, health facilities, housing options, transportation, exercise programs, advocacy, and more. DementiAssist: App for caregivers of dementia patients

The DementiAssist app assists the person caring for a person with dementia. It shows suggested actions to take if the person with dementia displays common triggered behaviors. To learn more or download the app, visit the app store for your mobile device and search DementiAssist.

Memory Café offers a social gathering

Each Café offers its guests the chance for unstructured conversation, refreshments and getting to know others in the community with similar memory changes. For more information call 757-220-4751 or see upcoming cafés.
