Professional Continuous Glucose Monitoring (CGM) (3-10 day wear)
This is wearing a professional (not a personal device) continuous glucose monitor (CGM) for 3 – 10 days pending your doctor’s order.
Certified Diabetes Educators will:
- Help you install the CGM device at the start
- Help you remove the device after the set time period
- Review the results with you and sends full report to the physician for follow up
This will allow your doctor to track how your blood glucose levels fluctuate across the day with food and activity. This can help your physician tailor your treatment plan and help you understand the influences on your blood sugar even if you do not have a regular CGM.
People with diagnosis of Type 1, Type 2 or Gestational Diabetes who’s physician has ordered the Profession CGM wear.

Initial visit to insert the device.
Second visit is longer and will include device removal and review of results. Then you will follow up with your provider for any treatment changes.
Appointments as needed
Covered by most insurance plans, including Medicare and Medicaid.

A physician referral is required for this class.
