What Is Pleural Effusion?

Pleural effusion is a condition where excess fluid builds up in the pleural space (the thin gap between the lungs and the chest wall). Normally, this space contains a small amount of fluid to help the lungs move smoothly during breathing. When too much fluid accumulates, it can cause breathing difficulty, chest pain, and other complications.
Pleural effusion is not a disease itself, but rather a sign of another underlying condition, such as heart failure, pneumonia, cancer, or pulmonary embolism.

Who Is Affected?

Pleural effusion can affect people of all ages, but it is more common in:

• Older adults
• Individuals with chronic lung or heart disease
• People undergoing cancer treatment

What Causes Pleural Effusion?

 Pleural effusions can be caused by many conditions. The fluid may be transudative (from pressure changes across the smallest vessels in the pleura) or exudative (due to inflammation or injury of the pleura). Common causes of transudative effusions include heart failure, liver cirrhosis or a type of kidney disease called nephrotic syndrome. Common causes of exudative effusions include pneumonia, cancers and inflammatory conditions.

Not Controllable

• Congestive heart failure
• Lung infections like pneumonia or tuberculosis
• Autoimmune diseases (e.g., lupus, rheumatoid arthritis)
• Cancer, especially lung cancer or breast cancer

• Smoking
• Poorly managed heart or kidney disease
• Occupational exposure to asbestos or toxins
• Lack of vaccinations for preventable infections (like flu or pneumonia)

• Individuals with heart, liver, or kidney disease
• Patients with cancer or recent chest surgery
• People with infections affecting the lungs

• Manage chronic conditions with your healthcare provider

• Avoid exposure to lung irritants and toxins
• Stay up to date on vaccinations
• Seek early care for infections or shortness of breath

What Are the Symptoms of Pleural Effusion?

Symptoms depend on how much fluid is present and the underlying cause. Common symptoms include:

• Shortness of breath
• Chest pain, especially when breathing deeply (pleuritic pain)
• Cough, often dry
• Fever or chills (if caused by infection)
• Difficulty breathing when lying flat
• Hiccups or reduced breath sounds (in large effusions)

How Is Pleural Effusion Diagnosed?

Your provider will review your symptoms and perform a physical exam. You may be referred for imaging and fluid analysis to determine the cause.

Common Diagnostic Tests:

• Chest X-ray: First-line imaging to detect fluid
• Ultrasound: Helps guide fluid removal and assess fluid volume
• CT scan: Provides detailed images of the lungs and chest cavity
• Thoracentesis: Procedure to remove and analyze pleural fluid
• Blood tests: To check for infection, inflammation, or organ function
• Thoracoscopy: If the above tests don’t produce a definitive diagnosis, a surgery under general anesthesia can be done to visualize the pleura, drain fluid and get some pleural biopsies to diagnose the condition.

How Is Pleural Effusion Treated?

Treatment depends on the cause, size, and severity of the effusion.

Lifestyle

• Stop smoking
• Manage chronic illnesses such as heart failure or kidney disease
• Follow a low-sodium diet if recommended for heart conditions
  
  

Devices

• Oxygen therapy may be used for breathing support
• Chest tubes or drains may be placed to remove fluid in recurring effusions
  
  

Medical Treatments

• Diuretics to reduce fluid if heart failure is the cause
• Antibiotics for bacterial infections like pneumonia
• Anti-inflammatory medications for autoimmune causes
  
  

Surgical or Interventional Options

• Thoracentesis: A needle drains the fluid and relieves symptoms
• Chest tube insertion: Temporarily used for continuous drainage in large or infected effusions
• Indwelling pleural catheter placement: mostly utilized in cancer related pleural effusion to help drain the fluid intermittently at home
• Pleurodesis: Procedure to prevent fluid from returning (often used in cancer-related cases)
• Surgery: May be needed to treat pleural tumors or remove thickened pleura in case of complicated pleural infections or empyema

• Pulmonary rehabilitation to improve breathing and stamina
• Palliative care for symptom management in advanced disease
