Activities of Daily Living - refers to the assistance an individual may need with eating, dressing, bathing, toileting, and ambulation.

Advance Directives - a legal document that outlines a resident's preferences for treatment should the person become unable to communicate his/her wishes. An advance directive may be in the form of a living will or a durable medical power of attorney. Our Director of Admissions or Social Services will be happy to answer questions and provide these documents to family members.

Adult Day Services - a great option for older adults who may benefit from socialization, support services, recreation, and meals in a supervised, social setting. With affordable daily rates, adult day services programs enable the resident to attend the program during the day and return home in the evening. Private funds, Medicaid, and other community options are sources of funding for the program.

Assisted Living - a relatively new option in long term care that provides assistance with activities of daily living to individuals who may need daily help with one or more activities of daily living. Residents receive round the clock medical supervision, delicious and nutritious meals, access to therapy services, and a multitude of daily activities to round out their days, nights, and weekends. Residents may bring special furniture and accessories from home to personalize their suites or rooms. Payment options include private funds and long term care insurance.

Hospice - highly specialized program for individuals who have a terminal condition. Caring and highly trained staff care for the resident as well as the family members. Intermediate Care - a nursing facility option that provides assistance with activities of daily living, social services, medication administration, rehabilitation services, and 24 hour nursing care. Private pay, long term care insurance, or Medicaid are options for coverage.

JCAHO Accreditation - a voluntary accreditation from the Joint Commission on Accreditation of Healthcare Organizations, an independent, non-profit organization. The Gardens at Warwick Forest is JCAHO accredited for long term care and subacute services.

Nursing Facility - a secure health care environment, this type of center provides 24-hour nursing care, rehabilitation services, meals, activities, as well as assistance with activities of daily living. Care may be provided on different levels including intermediate, skilled, or subacute. Payment for services usually includes private pay, long term care insurance, Medicaid, and Medicare (short term only).

Respite Care - temporary care that provides an opportunity for family members to take a break from caregiving with the comfort of knowing their elderly relative is receiving the medical care he/she needs. Respite care is available in adult day services, assisted living, or in the nursing facility setting. Coverage for respite care consists of private pay, Medicaid, and the Veterans Administration.

Skilled Care - a highly technical level of nursing and rehabilitation services generally reserved for individuals being discharged from a hospital or rehabilitation center. Skilled care includes a combination of nursing and therapy services designed to assist the patient in recovery and readiness to return home. The Medicare program provides coverage for eligible persons for up to 100 days in a Medicare certified unit. Medicare covers the first 20 days @ 100%; on day 21, the resident is charged a daily copayment; the copayment amounts vary from year to year and are generally covered by commercial insurances, Medicaid, or private pay.

Subacute Care - typically reserved for the most medically compromised residents. Specialized, short term services may include extensive wound care, cardiac or stroke rehabilitation, intensive rehabilitation, pain management, and medically complex care. The goal of subacute care is to enable residents to return to a lower level of care or home. Payment for services usually includes private funds, commercial insurance, or Medicare.
