Welcome

New Patients

For your first appointment, we ask you arrive 15 minutes early to get all your health information and insurance questions completed before your visit.

If your insurance plan requires a referral, you are responsible for obtaining a referral authorization from your primary care provider. If we have not received authorization prior to your visit, your non-emergent appointment may need to be re-scheduled.

Please bring the following to your first visit:

Your insurance card and ID
Referral forms and co-payments (if required by your insurance)
All current medication in their original containers
Copies of relevant diagnostic tests and medical records from anywhere outside of Riverside Health. If you had tests done at a Riverside facility or seen a Riverside Medical Group provider, we already have access to this information through the electronic health record, so no additional copies are needed.

If you need to change or cancel your appointment, please provide at least 24-hours’ notice before your scheduled visit. A $50 “No Show” fee may be charged if you failed to show for an appointment or no do not give adequate notice when canceling or rescheduling.
