Riverside Transfer Center
Riverside Transfer Center provides streamlined access to patient services for physicians. Through one phone number, physicians can transfer patients to the appropriate next level of care, consult with other physicians and request necessary medical transportation for their patients. When staying overnight at Riverside Shore Memorial Hospital, a hospital physician specialist - called a 'hospitalist' - manages the acute care needs of inpatients during their hospital stay. The hospitalist works with the patient and their primary care provider or specialty physicians to coordinate care until the patient is discharged. There are several advantages to having a hospitalist manage care:

  • Because hospitalists are on site around the clock, they are able to closely monitor a patient's condition and make treatment, testing or medication adjustments quickly.
  • The hospitalist is available to answer questions and communicate with the patient's family.
  • When discharged, the hospitalist will coordinate care with the patient's primary care physician to ensure a seamless transition back to outpatient care.
