New Patients: What to Bring to Your Appointment
Please bring all current medical insurance cards, referral if needed, a picture ID and a current list of medications to your first appointment. Please also bring relevant medical records and any related films, scans or other imaging studies. In order to complete registration, please arrive at least 15 minutes prior to your scheduled appointment.
