Chronic Disease and Diabetes Self Management is a Stanford-based program, a collaborative initiative between Riverside Center for Excellence in Aging and Lifelong Health, the Peninsula Agency on Aging, and CARE Coalition, for any adult with a chronic health condition and/or their caregivers.

Chronic conditions include any health issue that presents barriers to everyday life, such as:

  • Arthritis
  • Asthma
  • Cancer
  • Cardiovascular disease
  • Diabetes
  • Depression

The workshops include topics such as:

  • Pain management
  • Stress management
  • Physical activity
  • Communication techniques
  • Managing symptoms
  • Working with your health care team
  • Setting weekly goals
  • Effective problem-solving
  • Tips for eating well

There is no fee, just a commitment to 2.5 hours, once a week, for six consecutive weeks.

All participants receive a copy of the companion book, "Living a Healthy Life with Chronic Conditions," 4th edition.

CDSMP Leader training
Training for CDSMP Leader Workshops is FREE, but we do ask for a commitment after completing all four days to then co-facilitate one workshop in the following year with a trained leader through Coordination of a CARE Master Trainer. CDSMP workshops are 2.5 hours, once a week, for six weeks. This is a volunteer position in collaboration with 12 nonprofit agencies in the CARE Coalition.
