New Patients: What to Bring to Your Appointment
Please arrive 15 minutes early for your first appointment. Bring your insurance card, state-issued photo identification and, if needed, a copayment and referral (HMO patients). We also ask that you bring your list of medications with dosages or the original prescription bottles. If you have X-rays available, please bring them.
We don't require a referral, but some insurance providers require one from your primary care provider before you make an appointment with us. If you need help securing a referral, please call us and we'll be glad to assist.
