The My Healthy Lifestyle program gives employees with health insurance benefits through Riverside Health System and their covered spouses a chance to gain substantial savings on health insurance premiums or other health-related costs in 2013 and beyond.

It just requires a little effort on your part and some attention to the milestones built into the program. For example, keep these important dates in mind:

August 1, 2012
By this date, you should call your Primary Care Physician to schedule your annual wellness exam. Hopefully, you've taken care of that step. If not, you might still be able to get an appointment, so call right away.

August 31, 2012
The last day of the month is the deadline for bringing your 2012 Physical Exam Verification Form with you to the wellness exam so your doctor can complete it. You can also print one from the Riverside Intranet or from this link. Once your doctor completes the form, take it, and a copy of the lab results, with you. Be sure to make a copy for your records.

September 4, 2012 Postmark
This is the deadline for returning the 2012 Physical Exam Verification Form, along with a copy of your lab results, and your Patient Consent and Authorization, to Health Advocate. This information is required to determine if you qualify for the Riverside health plan discount.

What if I don't expect to meet the 30 point threshold for participation? Should I still have a wellness visit?
The answer is yes. Definitely yes. Even if you don't think you will meet the minimum 30 point threshold, we encourage everyone to get the screening done in order to get a baseline score this year. In this way you'll have a clearer picture of where you are right now with regards to your health. And you'll be able to talk with your primary care physician about your results, and what you need to do to make improvements in your health.  And it also makes sense in terms of the My Healthy Lifestyle program because you can achieve your health plan savings by reaching the minimum threshold or by showing a 20 point improvement over the previous year's score.

So even if you were to score 10 points this year, you can make changes and improve to 30 points by next year and get the full discount, by achieving a 20 point improvement over your baseline score. This is possible even though the threshold next year will be 50 points. But, if we don't have a baseline score from you this year, you will be expected to hit the minimum threshold next year of 50 points in order to qualify.

So meet the milestones, save on your premiums and make improvements in your health that will have lifelong benefits.
