Innovation and Lifelong Health
New approaches to healthier aging
When you consider that 10,000 Americans are turning 65 every day you realize that meeting their healthcare, lifestyle and community services needs won't be business as usual. That's why Riverside, which through its Lifelong Health Division is the state's largest health system based provider of services to older adults, has turned to an array of innovative new programs.
Senior Care Navigation
Sometimes, with all the choices, options and resources available, what you really need is a little bit of reliable guidance. Riverside's Senior Care Navigators can help, offering everything from medical care to every day needs like food and transportation and from counseling and support groups to help with living arrangements. This free service and the highly trained navigators who provide the human touch are ready to provide you and your loved ones with the information you need.
Tell us about yourself.
Riverside's PACE -- the Program of All-Inclusive Care for the Elderly – empowers participants to remain in their homes (rather than receive care in a long-term nursing facility) through the ongoing support of comprehensive medical and social services. Riverside currently offers five PACE programs serving seniors who live in metro Richmond, Petersburg and on the Virginia Peninsula.
Household Model of Care
This highly innovative approach to care replaces "institutions" with authentic homes while providing high quality clinical and other supportive services. These free-standing households, designed like traditional homes, provide long-term nursing care as well as short-term rehabilitation while supporting the lifestyles and choices of the individuals living in them.
