Post-Hospital Rehabilitation

Riverside Rehabilitation Institute is an accredited physical rehabilitation hospital offering in-patient recovery following serious injury, stroke, amputation and neurological conditions. |

For those recovering from illness, injury or joint-replacement surgery, Riverside’s Rehabilitation Centers are focused on returning you to your normal routine at home following 7 to 14 days (on average) of skilled nursing and therapy. |
| Outpatient Therapy |

Riverside offers physical, speech and occupational therapy at ten outpatient centers for those recovering from illness, injury or surgery. |
Community and In-Home Services
| ClearPath Memory Care |
 |
ClearPath is a specialized program dedicated exclusively to helping families, caregivers and loved ones manage memory impairment. ClearPath provides help and hope through newsletters, Memory Disorder Clinics and extensive resources for memory-related concerns. |
| Hospice |

Our Hospice program offers compassionate care to patients with a terminal diagnosis of six months or less and ongoing support for families and loved ones. |
| Home Health & In-Home Private Care |

Specially-trained nurses, aides and companions can provide skilled nursing to help you recover from an illness or injury or in-home private care to assist with everyday tasks such as dressing, cooking and cleaning. |

| PACE (Program of All-Inclusive Care for the Elderly) |

This unique program manages all care, medications, therapy and physician services for nursing home-eligible seniors who want to remain living at home. Participants enjoy activities and social programs at the PACE Centers. |
