What to Bring to your Appointment

You can make an appointment with your primary care provider using myHealth eLink. Riverside primary care providers have same-day appointments available most of the time. Remember, if your provider is not available, you may decide to visit another Riverside provider, who will have access to your electronic medical records creating seamless care.

What to bring
Although your Riverside primary care provider may have specific requests for what to bring to your appointment, generally, you will be asked to bring:
  • Insurance card
  • Identification (only needed for a first-time visit)
  • List of prescription and over-the-counter medications that you currently use
  • List of symptoms
  • List of questions
Before your first appointment, make sure that your provider accepts your insurance.

At your appointment
If instructions, words or anything your doctor says is unclear, please ask for additional explanation. We want you to feel comfortable about discussing all of
