New Patients

If you are a new patient, please arrive 15 minutes early to complete the necessary patient information forms or (dowload) print the following forms and complete before you arrive:

It is requested that all new patients bring the following to their first appointment:

  • Insurance Card
  • Referral forms, if required
  • Copies of pertinent diagnostic tests and medical records
  • All current medications
  • Completed history and physical form, if available

To ensure optimal care, please provide us with hospital records or results of GI testing at another facility. Please have copies of the reports sent to our office prior to your first visit.

We accept most major insurance as well as Medicare. Please call our office at (757) 599-6333 to verify that we accept your carrier.

Medical records
As a means of expediting care and increasing patient safety, we store medical records electronically. This secure system facilitates fast, clear communication among your physicians and specialists. In the event of an emergency, our physicians have immediate access to your medical history. This information can be critical and sometimes lifesaving in helping to properly diagnose and treat your condition. Please be assured that your personal medical records are confidential, and we will not release information without your permission. During your first appointment, and after we have established your electronic medical record, you may sign up for myHealth eLink. Through this secure, convenient, online communication tool, you can:
  • Access your electronic medical record summary
  • Request appointments and prescription refills
  • Review immunization records
  • Receive test results from your doctor
