- Financial Responsibility Form
- School Physical Examination and Immunization Record
- Birth Certificate
- Photo Release Form
- Signed HIPAA Sticker
- Payment of a $75 non-refundable registration fee
- Payment of a $75 yearly enrollment fee

Riverside Child Development and Learning Center strives to provide the very finest and rewarding child care services available in the area. The Center maintains an “open door” policy to promote parent/staff communication and encourage involvement in classroom activities and special events. Our role is to work in concert with the parent for the continued development of the child.
