You are eligible for Riverside PACE:

• If you are age 55 or older
• Able to live safely in the community
• Live in the Riverside PACE service areas: Peninsula: Newport News, Hampton, Poquoson or lower York County. Richmond: City of Richmond, Henrico, Chesterfield, Hanover and New Kent counties, as well as some areas of Petersburg, Powhatan, Goochland, James City and Charles City counties.
• Qualify for nursing home-level of care by Virginia UAI assessment

Virginia Uniform Assessment Instrument
A Virginia Uniform Assessment Instrument (UAI) is a form used by the Department of Social Services and the local Department of Health to assess an individual’s need for nursing home level of care.
