Wellness Coaching Registration and Health Risk Assessment Process for 2013 Premium Discounts

As of July 1, 2012, Riverside began offering a new Wellness Coaching program to employees and dependents covered by Riverside’s group health plans. Part of the My Healthy Lifestyle wellness program, this new Wellness Coaching program is offered in partnership with Health Advocate.

Our decision to offer the Wellness Coaching program reflects our interest in making certain that our employees have the best possible resources to help them take charge of their health. The following frequently asked questions focus mainly on Step 4 of the 2013 premium discount program, completing the Health Risk Assessment (HRA).

Q: I have already completed the Physical Exam Verification Form and mailed it to Health Advocate. Do I have to complete the Health Risk Assessment (HRA) to be eligible for health premium discounts in 2013?
A: Yes. You and your spouse, if your spouse is also covered under the Riverside group health plan, will need to complete the online HRA in order to receive the health premium discount in 2013.

Q: When do I have to complete the Health Risk Assessment (HRA) in 2012 to be eligible for health premium discounts in 2013?
A: To be eligible for health premium discounts in 2013, you and your spouse, if your spouse is also covered under the Riverside group health plan, needed to complete the online HRA between July 1 and November 30, 2012.

Q: How do I access the online HRA?
A: To access the online HRA, you will register yourself and your covered spouse with Health Advocate’s Wellness Coaching program available via the Health Advocate / Wellness Advocate portal at www.HealthAdvocate.com/riverside. When prompted to enter the name of your organization at the Member Log In screen, enter Riverside Health System. Q: What information will be required to register for Wellness Coaching via the Wellness Advocate portal?
A: You will be asked to verify your first name, last name, date of birth and zip code. You will also be prompted to create a unique user name and password.

Q: Do I need to provide an email address when I register for Wellness Coaching to complete the online HRA?
A: Yes, you and your covered spouse both need to provide a valid email address when you register. Your email address is used to complete your Wellness Coaching registration process and also to facilitate communication with your personal wellness coach going forward.

Q: What if I don’t have an email account?
A: If you don’t already have an email account, Google is just one of the service providers currently offering free email services. You may create a Google Gmail account for free by following the step-by-step instructions provided below.

Q: What information will I need to have available when I complete the HRA?
A: When you and your covered spouse are ready to complete the HRA process, you will want to have available your biometric values from your annual wellness exam.

Q: What if I don’t have my biometric values available when I complete the HRA?
A: If you don’t have your biometric values available, you can leave them blank initially and still complete the HRA. You will have the option to go back in and enter the values at a later date, up until the deadline.

Q: What if I don’t enter my biometric values by the deadline?
A: If you sent your completed 2012 Physical Exam Verification Form and lab results to Health Advocate, Health Advocate entered the results for you after the online HRA close date.

Q: I am not currently a Riverside group health plan participant. Can I complete the online HRA?
A: Due to cost constraints, the online HRA is only available to group health plan participants and covered spouses at this time.
