Step by Step Health Plan Savings
Earning Discounts on Your Riverside Health Plan Premiums
Riverside team members and their covered spouses who are enrolled in a Riverside health plan can earn discounts toward health insurance premiums. Both you and your covered spouse will need to complete these five steps to determine your eligibility for savings on your 2013 health insurance premiums.

STEP 1: |
Schedule your annual wellness exam with your Primary Care Physician. When you call, make sure to request the following lab work – a blood glucose test and a fasting lipid panel – so your doctor will have the results in hand when you come in for your scheduled wellness exam. It’s important to have your lab work completed before your wellness exam. Your wellness exam and lab work will be covered under Riverside health plans at 100%, with no co-payment. |

STEP 2: |
Be sure to bring your 2012 Physical Exam Verification Form with you to the wellness exam so your doctor can complete it. Once your doctor completes the form, take it, and a copy of the lab results, with you. Be sure to make a copy for your records. You’ll need it to enter into your Health Risk Assessment (HRA) as directed in Step 4. If your annual wellness exam was after July 1, 2011 and included all of the necessary medical information, you may use the results to fulfill the wellness program requirements. You will, however, need to have the medical provider complete the 2012 Physical Exam Verification Form and attach a copy of your lab results. |

STEP 3: |
Return the 2012 Physical Exam Verification Form, a copy of your lab results, and your Patient Consent and Authorization, to Health Advocate (3043 Walton Road, Suite 150, Plymouth Meeting, PA 19462). If you would like a pre-addressed, postage-paid envelope, call Total Rewards at (757) 534-5281. Health Advocate is a third-party resource (independent from your physician and Riverside health insurance), that holds – and keeps private and confidential – the medical information acquired through the exams. This information is required to determine if you qualify for the Riverside health plan discount. |
| STEP 4: Complete HRA |
Complete your online 2012 Health Risk Assessment (HRA). You will enter your results from your wellness exam as part of completing this HRA. The link to the HRA is active from July 1 through November 30. To access the HRA, go to www.HealthAdvocate.com/riverside.
Deadline: Complete this step between July 1 and November 30, 2012. |
| STEP 5: Follow Your Plan |
Work with your Primary Care Physician to execute your personal wellness plan through the year. Additionally, My Healthy Lifestyle will offer helpful information to move you forward on your road to better health. Riverside health plan participants can access Wellness Advocate, a new service available through Health Advocate. |
Reaching Your Health and Wellness Goals
To receive savings on your health insurance premiums for plan year 2013, you and your covered spouse (if applicable) must complete the steps above. Your discount is based on whether or not you meet the requirements outlined in the chart below.

How to Earn Points
Team members and covered spouses accrue points based on health risk factors, including tobacco use, body mass index (BMI), blood pressure, cholesterol, and glucose.
