Enroll within 30 days
All benefits-eligible employees (full-time, part-time, and pay partial) need to complete Online Enrollment within 30 days of their official start date by logging on to Employee Self Service in Lawson. You can log on from any Riverside network computer. You may also enroll from home (go to the secure Employees section of the site and enter the password. Look for the ESS link.)

What you need to do:
1. Review plans. Links appear in the table below.

2. Consider rates. Find out what a plan will cost you by logging in to Employee Self Service in Lawson. Also consider the Health Insurance Marketplace, a government-regulated website where anyone can buy insurance and some people may qualify for a subsidy to make it more affordable. Remember that everyone is required to have insurance in 2014 or pay a penalty. Click here to learn more about how the Marketplace works.

3. Enroll. Do that at Employee Self Service.

2014 plan information

Benefit Features
CareAdvantage
PPO
Deductible $500 per individual, $1,000 per family. Riverside Platinum network co-insurance 10%.
Lumenos High-Deductible Health Plan (HDHP) Deductible $1,500 for individual, $3,000 for family.
Prescription coverage PPO plan is Premier/MedImpact. High-deductible plan uses ExpressScripts.
Flexible Spending Accounts Medical Flexible Spending Account annual contribution limit is $2,500, Dependent Care Flexible Spending Account annual contribution limit is $5,000.
Health Savings Account (for HDHP enrollees) Contribution limits are $3,300 for an individual, $6,500 for a family. (Participants 55+ may contribute an additional $1,000)
Dental Optional coverage.
Life and disability Provided by Lincoln.
Aflac Extra coverage to fill gaps.
Legal Resources Advice, consultation and more.
Spousal surcharge $200 per month.
Health Advocate Wellness coaching and advocacy for plan participants and family members, including parents and in-laws
My Healthy Lifestyle Premium discount incentive of up to $750
