JOB SUMMARY: Performs a variety of duties related to cardiac electrophysiology procedures; cardiac ablation for the treatment of atrial and ventricular arrhythmias, implants for cardiac rhythm management (CRM) devices, and ongoing CRM maintenance. Works independently in a flexible work schedule, giving continued clinical support thru the practice CRM clinic for interrogation and programming implanted devices. We are seeking dedicated Universal Care Partners to join our team at PACE MACTAVISH Day Center to care for geriatric clients in our community!PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, home based emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services. To work primarily Home Care patient assignments/cases through the PACE-MacTavish facility with rotation to the PACE-MacTavish Day Center and Riverside Senior Care Center Day Center as needed to meet patient care needs.EDUCATION:High School graduate or GED equivalent required. High School graduate or equivalent.Graduate of State of Virginia approved Nurse Aide Training Program.LICENSURE:Licensed by the State of Virginia as a Certified Nursing Assistant (CNA) required. Proof of current, valid driver's license and acceptable driving record required upon hire and to be maintained during employment. PACE is a community-based program which provides participants with all necessary health services while they stay in their homes.It covers many different types of services including a day center, personal care in the home; home based emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services.

The Universal Care Partner (Certified Nursing Assistant) works under the general supervision of the Home Care Services Manager and Day Room Supervisor. The position performs tasks in the PACE Day Center and the Participant's home which support the social, physical and cognitive functioning of seniors with impairments. The Universal Care Partner demonstrates the knowledge and skills necessary to provide and/or assist with appropriate care to include: personal care (feeding, grooming, bathing, toileting), home services to include laundry, memory stimulation activities (social, recreational), motor activities, escorting to medical appointments, shopping, assisting with transportation of participants to and from the facility utilizing the facility transportation vans and other activities as directed by the Participant's plan of care (POC). Assists with any identified needs of the participant in their home that will enable the Participant to perform at the greatest level of independence possible.

EDUCATION:High School graduate or GED equivalent required. Must also be registered by the Virginia Department of Health Professions as a certified nurse aide.

LICENSURE:Licensed or license eligible by the State of Virginia as a Certified Nursing Assistnat (CNA) required.Proof of current, valid driver's license and acceptable driving record required upon hire and to be maintained during employment.CPR Certification and First Aid training must be obtained within 30 days of hire and maintained throughout employment.

EXPERIENCE:Minimum one (1) year experience working with the frail and/or elderly population required.Previous experience as Nurse Aide preferred.

HOURS:Ability to work varying shifts (day shift, early evenings and weekend shifts) based on participant's home care needs.
