The Wound Ostomy Continence Nurse is responsible to the Director Clinical Operations HCD to assess, plan, make nursing diagnosis, implement and evaluate care for clients with fecal or urinary diversions; clients with draining wound problems, pressure ulcers, and clients with bowel or urinary incontinence. The WOCN provides continuity of care of the ostomy, incontinent, and skin care client by improving the competence, performance, and understanding of the nursing staff. The WOCN functions as a consultant for clients with complex draining wounds, fistulas, pressure ulcers, incontinence, and skin problems related to fecal and urinary incontinence. The WOCN serves as a resource person for nursing, allied and medical staffs, client, family and community. In addition, this position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the client served. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the client's status and to provide or direct the care needed as described in the departmental policy and procedure guide. The individual is responsible for maintaining an ongoing plan for the nursing staff which will include orientation, in-service training and continuing education. Duties of the WOCN include: Evaluates client's response to care and updates the plan of care appropriately. Recognizes changes in client status and initiates action. Incorporates client/family rights into nursing practice. Engages in client/family teaching. Seeks to meet learning needs in order to maintain competency.
