This position will be responsible for a designated group of PACE participants in the promotion and maintenance of health through teaching, counseling and appropriate preventive and rehabilitative services. The Clinical Care Coordinator is responsible for assessing the health needs and contributing to the plan of care, implementation of nursing care plans and providing nursing care as well as periodic revaluation of the PACE participants nursing needs. Including; evaluating the participants response to care and making recommendations for changes to the plan of care as appropriate. The Clinical Care Coordinator engages in participant and caregiver teaching as appropriate and effectively communicates with participant, caregivers and PACE interdisciplinary care team. This role demonstrates the knowledge and skills necessary to assess, plan care for and provide service to frail elderly participants according to assigned responsibilities and Riverside PACE standards. The Clinical Care Coordinator assists in PACE facility medical clinic as scheduled.
EDUCATION: Graduate from an accredited school of nursing as a Registered Nurse required.

The Clinical Data Abstractor works under the direct supervision of the Manager of Quality and Performance Improvement. The Data Abstractor reviews clinical documentation and ensures accuracy and completeness of clinical information used for measuring and reporting process measures and medical necessity as appropriate to specific job function. This individual is responsible for data abstraction and the analysis of clinical data from a variety of sources to promote performance improvement initiatives and maintain compliance with Joint Commission and other regulatory standards. QUALIFICATIONS:
EDUCATION: BSN preferred.

EDUCATION: A) Holds a certificate of completion from an approved nurse aide training program and be registered by the Virginia Department of Health Professions as a certified nurse aide or B) Hold a certificate of completion from a professional nursing practical nursing or nurse aide training program approved by the Virginia Department of Education or the Virginia Community College System or the Virginia Department of Health Professions or C) Holds a certificate of completion from a home health aide training program approved by the Department of Health prior to July 1, 1984 and have been in continuous practice since that time. The Registered Nurse will work labor-pool from 7a-730p or 7p-730a to fill in as needed and to include every other weekend and rotating holidays. The Oncology Unit is an 18 bed Medical Oncology unit that specializes in the care of the oncology patient and family. The units designed and operating structure accommodate the complex needs of patients with cancer-related emergencies stemming from a variety of oncologic disorders. There is a special lead-lined room on the unit used for cesium implants and radioactive iodine therapy. Nurses provide care for newly diagnosed patients, those receiving cancer treatments such as chemotherapy or radiation, those receiving care for symptom control, both from disease or the side-effects of treatment and those needing pain relief through palliative care.

The Women and Infant Care Center provides spacious rooms featuring private restrooms. The majority of our babies will remain in the rooms with mom at all times and has the same nurse to provide couplet care. The RN On-Call provides support to the agency after hours via phone or patient visit as necessary. Will take calls from patients regarding issues or concerns and triage the clients needs as to whether a visit is necessary or if the problem can be handled via patient/caregiver education via telephone. Will be responsible for taking any after hours referrals and forwarding information to the Care Manager on call. The Wound Ostomy Continence Nurse is responsible to the Director Clinical Operations HCD to assess, plan, make nursing diagnosis, implement and evaluate care for clients with fecal or urinary diversions; clients with draining wound problems, pressure ulcers, and clients with bowel or urinary incontinence. The WOCN provides continuity of care of the ostomy, incontinent, and skin care client by improving the competence, performance, and understanding of the nursing staff. The WOCN functions as a consultant for clients with complex draining wounds, fistulas, pressure ulcers, incontinence, and skin problems related to fecal and urinary incontinence. The WOCN serves as a resource person for nursing, allied and medical staffs, client, family and community. In addition, this position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the client served. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the client's status and to provide or direct the care needed as described in the departmental policy and procedure guide. The individual is responsible for maintaining an ongoing plan for the nursing staff which will include orientation, in-service training and continuing education. Duties of the WOCN include: Evaluates client's response to care and updates the plan of care appropriately. Recognizes changes in client status and initiates action. Incorporates client/family rights into nursing practice. Engages in client/family teaching. Seeks to meet learning needs in order to maintain competency. Submits daily visit and travel records with visit documentation on a daily basis. Provides Director of Clinical Operations with a weekly schedule of activity. Complies and submits monthly reports detailing referrals, client visits and education activity.
