Responsible for payment authorization of agency provided care, Faxes referral and plan of care to contract agencies and coordinates aide services, Assists with orienting PACE aides to new participants and reviewing plan of care; responsible for assisting in follow up regarding concerns and issues relating to aides and keeping home care manager informed of same, communicates requests for changes in the plan of care from participants to home care manager; participates in case conferences as required. High school diploma or equivalent required. Certified Nursing Assistant preferred. Minimum of 1 year experience with a frail and elderly population required. Experience with scheduling two years highly preferred. Previous home care/private duty/personal care experience preferred. Satisfactory background check required per licensing.
