Home Care Manager, RN

PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, home based emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services. JOB SUMMARY:Works under the supervision of the PACE Senior Nurse Manager, oversees the day to day operations of the PACE In-Home Services program. Responsible for all in-home services staff, including RNs, LPNs, Schedulers and Office Assistants. Assists leadership in the policy formation, program development and implementation and annual budgeting. Provides supervisory visits to home care staff (RN, LPN, UCP) in the field to evaluate skills, customer service and general performance. Assists with initial, annual and change in condition assessments as needed in the participants home environment. Coordinates plan of care with appropriate community resources and the IDT. Acts as the liaison between any contract agencies and PACE, including facility settings such as assisted living and nursing facilities. When appropriate, provides direct care and education to participants and families in the home setting and documents accordingly in the medical record. Communicates changes in participant condition and progress to the IDT and Home Care team members. In the absence of the Sr. Nurse Manager may function in that role as requested by the Site Director. Will be responsible for the development, implementation and oversight of the Mental Health Program for Riverside PACE. Will provide individual, family and marital therapy services by referral to PACE facilities and collaborate with social work staff, medical staff, and other disciplines as needed to assess and address the emotional and psychological needs of participants. Will work with other contracted mental health agencies to coordinate additional mental health care as needed. Will facilitate educational in-services to PACE staff on mental health topics as needed. Will provide managerial support, oversight and supervision to PACE social work department at designated PACE site.

Blue Ridge PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, home based emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services. PACE located in Charlottesville, VA is seeking a full time Senior Nurse Manager.The Senior Nurse Manager works under the general supervision of the Site Director. Responsibilities include daily operational and management oversight of the PACE Center, direct operational oversight of Home Care and Clinic Services and all care provided by nursing staff. Ensures the completion of the initial, six month and annual nursing assessments of participants by registered nurses as well as the development of the nursing Plan of Care (POC) for each participant. The Senior Nurse Manager is responsible to ensure that continued monitoring of the health needs of PACE Participants is being performed and met by all nursing staff in the home, center and community. Serves as a member of the PACE Interdisciplinary Team (IDT) and regularly attends scheduled meetings. As the senior clinical leader for the Center, in cooperation with the Physician is responsible for clinical and utilization management oversight for all participants. Responsible for oversight of referrals to outside medical appointments for PACE participants are coordinated and services have been authorized.
