Clinically sophisticated, yet intimate and comfortable, The Gardens at Warwick Forest was one of the first centers in the area to provide Medicare skilled services. The 60-bed medicare skilled unit provides RN coverage 24 hours a day, a full time Nurse Practitioner and a team of dedicated physicians, nurses, aides, social workers, and dietitians. In addition, a full time chaplain oversees the Pastoral Care program and directs interdenominational services in the welcoming Chapel. The subacute unit specializes in wound management, intravenous therapy, rehabilitation, and complex medical care. In addition, a specialized memory impaired unit for individuals with minimal to moderate dementia provides a structured program in a calm, comfortable, and safe environment. We provide 24-hour nursing care. Accommodations throughout the Center include tastefully furnished private and semi-private rooms, courtyards, lounges, dining areas, and a beauty shop. Monthly support groups are provided to enhance family education and involvement.

This position works under the general supervision of the PACE Site Director. Responsibilities include completion of the initial, six month and annual nursing assessments of participants and the development of the nursing care plan and ensuring the plan is initiated. The clinic nurse is responsible for monitoring and evaluating the health needs of PACE participants and recognizing changes in participant status and initiating appropriate actions. Evaluates participants response to care and updates the plan of care as appropriate by documenting in the electronic medical record. Engages in participant and caregiver teaching as appropriate and effectively communicates with participant, caregivers and the PACE interdisciplinary care team. Responsible for management and oversight of PACE nursing staff and universal care aides (PCA) at the day center. The clinic manager is responsible for monitoring medication administration to participants at the facility. Responsible for the daily operation of the PACE clinic to include maintaining adequate supplies and overseeing physician schedules. The clinic manager is responsible for ensuring referrals to outside medical appointments for PACE participants are coordinated and approved for payment.
Requirements
EDUCATION: RN Graduate of a Professional School of Nursing; diploma required. Will accept Associates, Bachelors or Masters preferred.
LICENSURE: Current licensure as a RN in the state of Virginia in good standing and without restriction. CPR certification required. Current BLS certification preferred; required within 3 months of hire.
EXPERIENCE: Minimum of two years required in a previous position of Nurse Management with an emphasis on experience in an acute, clinic or skilled care setting, three to five years experience preferred. Minimum of 1 year experience with a frail or elderly population required.

Represent Riverside Health System Foundation at community events in assigned territory. Prepare donor proposals, initiate calls and conduct in-person visits to solicit major gifts. Responsible for conducting initial recreational therapy assessments on potential participants using techniques such as observation, review of medical records, standardized assessment tools and reports from participant and/or family members to identify their recreation and leisure interest and level of social interaction. Conducts on-going assessments to include at a minimum six month and annual assessments on current participants. Updates the recreational plan of care as appropriate. Develops and implements general recreation and/or therapeutic recreation programs and goals as part of the plan of care. Produces monthly recreational calendar for PACE center. Helps with responsibility for day-to-day operations for fleet of specially equipped vans for transporting adults to PACE site, back home, and to appointments or outings. Helps plan, develop, communicate, and monitor transportation routes, Drivers Schedules and participant program pick-up/delivery assignments, ensures vehicle maintenance is scheduled and completed. Helps coordinate arrival and departure times and schedules with other departments.
