Clinic Manager (RN)
Riverside Health System has an exciting opportunity for an RN to work as the Clinic Manager at our PACE (Program for All-Inclusive Care of the Elderly) facility off of MacTavish Ave in Richmond, VA. This position works under the general supervision of the PACE Site Director. Responsibilities include completion of the initial, six month and annual nursing assessments of participants and the development of the nursing care plan and ensuring the plan is initiated. The clinic nurse is responsible for monitoring and evaluating the health needs of PACE participants and recognizing changes in participant status and initiating appropriate actions. Evaluates participants response to care and updates the plan of care as appropriate by documenting in the electronic medical record. Engages in participant and caregiver teaching as appropriate and effectively communicates with participant, caregivers and the PACE interdisciplinary care team. Responsible for management and oversight of PACE nursing staff and universal care aides (PCA) at the day center. The clinic manager is responsible for monitoring medication administration to participants at the facility. Responsible for the daily operation of the PACE clinic to include maintaining adequate supplies and overseeing physician schedules. The clinic manager is responsible for ensuring referrals to outside medical appointments for PACE participants are coordinated and approved for payment. The Transportation Coordinator works under the general supervision of the Transportation Manager and provides leadership to the Drivers. Helps manage transportation department and helps supervise staff. Participates in daily IDT meeting. Helps with responsibility for day-to-day operations for fleet of specially equipped vans for transporting adults to PACE site, back home, and to appointments or outings. Helps plan, develop, communicate, and monitor transportation routes, Drivers Schedules and participant program pick-up/delivery assignments, ensures vehicle maintenance is scheduled and completed.
